Put {{Guildcat}} at the top of server category pages, e.g. Category:Mal'Ganis US guilds.
The template should automatically determine the correct server name from the page name of the guild category. If this does not work for some reason, {{Guildcat|<Server:Servername>}} can be used to specify the server name.
